My acceptance into the ITALIAN GAY OPEN 2004 is without assumption of responsibility of any kind by the Associazione Tennisti OMOsessuali (known as A.T.OMO.) or its officers, the Gay and Lesbian Tennis Alliance (GLTA) or its officers, the Tennis Corvetto or any public or private facility, sponsors, host club, tournament Director, or individuals and organisations connected with the A.T.OMO. I agree to waive any and all rights and claims for bodily and emotional injury, damage to my reputation, or any other arms I might suffer. I also waive any and all rights to claim from the A.T.OMO, or any of the above organisations or individuals, damages which may arise as a result of references or photographs of me, which may appear in publication, any media, or internet coverage of the tournament. I understand this tournament will govern the conduct of the players and enforce the GLTA Rules and Procedures and the FIT (Italian Tennis Federation) rules and I agree to conduct myself accordingly. Proper attire will be worn in accordance with Tennis Corvetto. Accordingly to the Italian Law on Privacy (D. Lgs 196/2003). I give my authorisation to the handling of my personal data.
Dichiaro di sollevare da responsabilità d'ogni tipo l’Associazione Tennisti OMOsessuali (A.T.OMO) ed i suoi rappresentanti, la Gay and Lesbian Tennis Alliance (GLTA) ed i suoi rappresentanti, il Tennis Corvetto, così come il Direttore del torneo e tutti gli sponsor, le associazioni e gli individui connessi all’A.T.OMO. Rinuncio inoltre a qualsiasi diritto e pretesa derivanti da infortuni fisici, da danni alla mia reputazione e da qualsiasi altra lesione che potrebbe derivarmi dalla partecipazione al torneo. Concordo nel rinunciare a tutti i diritti vantati nei confronti dell’A.T.OMO. e delle sopra citate organizzazioni ed individui per i danni emergenti dall’apparizione della mia immagine o di mie dichiarazioni su stampa e qualsiasi altro mezzo di comunicazione, comprese le pagine internet del torneo. Ho presente che il torneo seguirà le regole e le procedure della GLTA e della Federazione Italiana Tennis e dichiaro, pertanto, di comportarmi secondo tali disposizioni. Il mio abbigliamento sarà conforme alle norme del Tennis Corvetto. Autorizzo l’A.T.OMO. al trattamento dei miei dati personali ai sensi del D. Lgs 196/2003.
